Even looking at when she kills Shion, she was also very calm. When she kills Kimiyoshi, she wasn't manic. Her brief psychotic trance is not as hysterical as Shion in many cases. Mion, while outgoing and outspoken, was very calm and collected. Shion, while looking sweet, can often have emotional outbursts like hitting Keiichi with the chair or hitting Satoko when they're in the way of what she wants. The biggest difference is how the curse affect them as well. Basically, Mion would murder for the sake of those she holds dear and plans to end her own life to take responsibilities for her actions while Shion murder for herself and to escape death. On the other hand, Mion will kill herself after killing everyone involved with the curse, and her motivation is also to protect someone. Her main motivation after the festival is killing everyone, so they wouldn't kill her. She pushed the blame on Satoko, she ran away from the school, blamed Satoko, she laid out her family secret without a thought which led to her fingernails being removed then held a resentment toward them, she went from revenge to trying to away with murder. While Mion is dutiful, Shion often tries to run away from her responsibilities or when things get hard and pushes the blames and responsibilities on someone else. While at the academy, she did not just simply leave, Shion decided to screw with the school by spreading rumor about a student/faculty relationship. She beats up Satoko and blaming her for something that is outside of her control.
When Satoko is being abused by her aunt and uncle, she sees Satoko as an inconvenience like she would want her to disappear then her problems with Satoshi would be solved. Even before the curse, she tends to blame other people around her, and she can be a bit selfish.
When Shion has to take responsibilities for the mistakes by having her fingernails removed, Mion did it on herself as well. She takes it upon herself to protect Keiichi and Rena in Tatari and Tsumihoroboshi-hen arc by hiding the body. She has a very selfless nature and strong mental fortitude. Mion as the head of her house has always tried to take responsibilities for her action, and she has a strong sense of duty to her friends and family. The biggest reason would be their sense of responsibilities. Mion would never get the Hinamizawa's syndrome under any normal circumstances while Shion with very simple triggers.
When you look back at Mion and Shion and the Cotton arc between the 2 series, you can see a big contrast and even how they deal with L5 disease.
